Requesting My Medical Records

At One Life Counseling Center, we respect each client’s right to access their health information and records. We also take confidentiality seriously and work carefully to protect your privacy throughout the records request process.

Who Maintains Your Records

If you see a Trainee or an Associate therapists:

Your record is maintained by One Life Counseling Center. We can help with copies of your records or with sending them to another person or organization. (see forms below)

If you see a Licensed clinicians: Your clinician maintains their own records and is the legal custodian of those records. One Life does not have access to, maintain, or release records for independently contracted licensed clinicians. Please contact your Licensed clinician directly; their contact information is in our staff directory.

How to request your records?

*this applies only to records maintained by One Life for services provided by trainees and Associates.

You may request records if you are the client, the parent or legal guardian of a minor client, or an authorized representative with appropriate legal authority. Access and release are subject to applicable law and may depend on the specifics of the request. 

  • Copy for yourself: Complete our Written Request for Records Form. Include your name, date of birth, therapist's name, and approximate dates of service if known. We may need to verify your identity before releasing records. Where to send form info is to the right under “Where do I submit my Request forms”.

  • Sending records to a third party, or requesting on someone else's behalf: Complete an Authorization for Release of Information (ROI) form: English | Spanish. Include the client's identifying information, what's being requested, the purpose, and your preferred method of receiving records.
